Grupo 1: Grupo 1: Mechanical ventilation and respiratory mechanics
Mechanical ventilation supplies the tidal volume that the patient's respiratory muscles can no longer generate adequately. The pressure required to deliver that volume rises when respiratory-system stiffness increases or airway resistance increases. Plateau pressure reflects the pressure needed to hold the delivered volume without continuing airflow, while the difference between plateau pressure and end-expiratory pressure is monitored as a lung-stress quantity; the narrative identifies a protective target below about 15 cmH₂O for this difference.
Grupo 2: Grupo 2: Dialysis and artificial solute clearance
Dialysis removes waste solutes from blood through an artificial membrane, replacing renal excretory function. Session adequacy depends jointly on the artificial filter's clearance of a reference solute, dialysis duration, and the patient's distribution volume for that solute. These three quantities combine into a delivered-dose index, and the narrative states that a minimum reference value separates adequate from insufficient dialysis without specifying that numerical threshold.
Grupo 3: Grupo 3: Extracorporeal membrane oxygenation
Extracorporeal membrane oxygenation withdraws blood, oxygenates it through an artificial membrane, and returns it to the patient's circulation. Oxygen supplied by the system increases with extracorporeal blood flow and with the oxygen-content increase produced across the membrane; configuration differs depending on whether cardiac support, pulmonary support, or both are being substituted. The fraction of total oxygen consumption covered by the circuit increases with extracorporeal contribution and can reach roughly three quarters in the illustrative severe-lung-damage case described in the source.
Grupo 4: Grupo 4: Parenteral nutrition and nitrogen balance
Total parenteral nutrition bypasses gastrointestinal digestion and absorption by delivering macro- and micronutrients directly into a central vein. Nutritional adequacy is summarized by nitrogen balance: nitrogen intake from supplied protein is compared with nitrogen excretion over the same period. A substantially negative balance, of several grams per day, indicates severe endogenous protein catabolism; increasing protein supply aims to move this balance toward zero or slightly positive during recovery.
Grupo 5: Grupo 5: Ventilator weaning and transfer of respiratory work
Ventilator weaning progressively transfers respiratory work from mechanical support back to the patient's own respiratory muscles. Readiness is assessed by combining spontaneous respiratory frequency with the tidal volume generated by the patient: rapid breathing with small tidal volumes predicts weaning failure. During weaning, pressure support is gradually reduced while patient work is monitored; excessive respiratory frequency, unsafe oxygenation, or visible agitation triggers failure and return to full mechanical ventilation.
Grupo 6: Grupo 6: ECMO weaning and recovery of native function
ECMO weaning is governed by the balance between extracorporeal oxygen support and recovery of native cardiac or pulmonary function. As native oxygen delivery recovers, the fraction that must be supplied by ECMO falls, permitting gradual reduction of extracorporeal support. The source defines this dependency qualitatively but does not provide a unique withdrawal law or quantitative readiness threshold.
Grupo 7: Grupo 7: Integrated intensive-care life-support control
Intensive care combines several forms of artificial support while continually checking whether each one remains adequate, safe, and necessary. Lung-pressure monitoring constrains mechanical ventilation, delivered dialysis dose constrains renal replacement, nutritional balance tracks catabolism, and recovery/readiness measures determine when ventilator or ECMO support can be reduced. The integrated control objective is therefore to sustain failed physiology without causing avoidable support-related harm and to withdraw artificial work as native function returns.
